After Sunday night’s loss to the New England Patriots, it’s fair to say that the Buffalo Bills now have a Matt Milano problem.

Ever since Milano achieved All Pro status and made it to the Pro Bowl in 2022, the ninth year linebacker has had the worst of luck with an inability to stay on the field.

It’s been a constant battle with injuries for a player who the Bills used to rely heavily on. Now it’s just good fortune if he’s ever available.

During Week 5 of the 2023 season, Milano was well on his way to having another career season, until he suffered a season-ending fractured leg.

As he made his comeback in 2024, he then suffered a torn bicep in his fourth game of the season.

And now, in 2025, Milano keeps bouncing on and off the injury report, as he’s currently dealing with a pectoral injury, so much so that he had to leave Sunday night’s game against the New England Patriots because of it.

After Week 2 against the New York Jets, where Milano originally aggravated his pec, coach Sean McDermott expressed how disappointing it was for both the player and the team that he continues to deal with these issues.

“He's one of our better players. And so we'll see where it goes. I feel bad for him… He works extremely hard. You guys know him, he's here quite a bit, and he puts a lot of time into it, and it means a lot to him."

Yesterday in his Monday press conference, coach Sean McDermott gave an update on Milano and it sounds like they are taking it on a week-to-week basis.

"He tweaked that pec in the game yesterday, so he'll be week to week… Nobody works harder than Matt Milano to stay healthy," McDermott said. "Right now it's going to be next man up for this week."

From 2019 to 2022, Milano was one of the league’s better, yet underappreciated linebackers. In total, he combined 331 tackles which may not have been in elite linebacker territory, but it was in highly efficient territory.

And that’s the type of player Milano has always been – efficient.

The mounting injuries are certainly a concern for the veteran linebacker, and if it continues to limit him, it’ll be on Shaq Thompson to step in and assume control.
